Bridge to Wellness: Care Management
With Bridge to Wellness, we deliver mental health and substance use services to individuals and families in all communities through a person-centered approach. We provide care management, peer support, and direct connections to care and recovery resources. By partnering with local organizations and addressing both clinical and everyday challenges, we help people access the support they need to achieve stability, wellness, and long-term recovery.

Recommend and refer
We have partnered with Sayin, a healthcare resource platform, to enable us to more quickly and efficiently recommend high quality resources and care to our clients. We have many affiliated partners that we refer to more often but we refer primarily based on the best clinical fit for each individual.

Connect to local resources
Through our affiliation programs and the Sayin platform, we connect individuals to free or low-cost local resources like mental health support, recovery programs, housing help, and job training. By partnering with community groups, faith-based organizations, and agencies, we simplify access to services that match each person’s needs—removing barriers and guiding them toward recovery and stability with confidence.

Assessment
Navigating all of your options is scary and time consuming. We offer services that enable you to talk through your desires and expectations for your treatment. We then prepare you by talking over what to expect and how to adjust yourself and expectations for the best experience possible.

Payment assistance
Depending upon funding, we also offer payment assistance to enable those at risk of losing or not receiving the care that they need due to times of economic hardship. We understand that financial limitations are one of the leading barriers to entry in behavioral healthcare for underrepresented groups and minorities.

Focus communities
Focus communities, are unique and are enabled via the Sayin platform. They enable you to communicate and engage with specific groups of people that are dealing with or have undergone similar situations as yourself. Creating a strong interconnected community in safe and moderated spaces.
